This full-time, fully remote position involves conducting patient outreach, care coordination, clinical documentation, and nursing support services. The ideal professional for this job is culturally sensitive, compassionate, clinically experienced, and capable of communicating effectively in both Spanish and English.
Key duties include connecting with Spanish-speaking and English-speaking patients, explaining treatment plans and medications, documenting interactions in EMR or EHR systems, and escalating clinical concerns appropriately. Candidates must possess an active New York nursing license and be available during designated United States business hours.
Responsibilities
- Perform patient outreach and follow-up communications in Spanish and English
- Deliver nursing support, patient education, and care coordination services
- Interact with patients, caregivers, providers, and healthcare teams about treatment plans
- Record patient interactions thoroughly and accurately within EMR and EHR platforms
- Help patients navigate medications, appointments, healthcare resources, and treatment plans
- Handle assigned activities related to case management, utilization review, intake, or clinical support
- Escalate clinical concerns correctly and maintain compliance with HIPAA and organizational policies
- Work alongside interdisciplinary teams to foster positive patient outcomes
- Engage in quality improvement and team projects
Requirements
- Active and unrestricted Registered Nurse license in New York
- Bilingual fluency in written and spoken Spanish and English
- At least 2 years of clinical nursing experience
- Strong critical-thinking and clinical assessment capabilities
- Proficiency with Electronic Medical Records and Electronic Health Records systems
- Capacity to operate independently in a remote setting
- Availability to work during designated US business hours
- Reliable internet connection and a dedicated workspace
Qualifications
- Additional active US state nursing licenses
- Background in care coordination, case management, telehealth, utilization review, managed care, or patient intake
- Experience interacting with diverse patient populations
- Prior remote healthcare work experience
- Bachelor of Science in Nursing degree
