Job Description
Evergreen Nephrology is hiring a bilingual Care Coordinator who holds an LPN license for its Connected Care Program. The company works with nephrologists on value-based kidney care, aiming to slow disease progression, move care into the home and speed up kidney transplants. The job is fully remote and runs on Pacific Time, from 8:30am to 5pm.
The coordinator conducts monthly chronic care management check-ins for an assigned patient panel. Before each call, the nurse reviews diagnoses, kidney history, medications, labs, imaging and treatment cycles. During calls, the nurse runs structured symptom checks, looks at medication adherence and side effects, flags warning signs and escalates urgent concerns through defined triage pathways.
The coordinator also reinforces the provider's care plans, explains symptom management within LPN scope, helps arrange follow-up visits and labs, and looks for social or access barriers. Every interaction is recorded in the EHR and the CCM platform, qualifying time is logged, and clinically relevant messages are routed to providers. Targets include at least 80 percent monthly engagement and 90 percent billable conversion.
Candidates need an active California LPN or LVN license, fluency in Spanish and English, and one to two years of hands-on work in outpatient care, population health, care coordination or case management. A private workspace is required to protect health information, along with home internet of at least 25 Mbps download and 10 Mbps upload. Evergreen supplies telephony applications and equipment.
Pay is 55,000 to 65,000 USD a year, with a quarterly bonus.
