This role is well-suited for licensed Registered Nurses who bring background knowledge in telehealth, case management, or maternal health and seek a flexible work arrangement. The position requires approximately 24 hours per week, featuring self-managed hours coordinated with team coverage goals and business needs.
The person in this role will review patient metrics like symptoms, weight, and blood pressure to spot high-risk situations, deliver clinical evaluations through messaging, phone, or telehealth, and educate individuals on postpartum recovery, preeclampsia risks, gestational diabetes, and hypertension. Additional duties include connecting patients to resources, handling referrals, coaching on remote patient monitoring adherence, escalating critical cases according to established protocols, and recording clinical interactions within the Babyscripts care management platform.
Responsibilities
- Review patient data trends such as blood pressure, weight, and symptoms to flag high-risk situations
- Conduct clinical assessments through phone, telehealth, or messaging to deliver interventions and education
- Educate patients directly regarding maternal health concerns, postpartum recovery, preeclampsia risks, and gestational diabetes
- Escalate high-risk cases to appropriate providers using established protocols
- Aid in care coordination by organizing follow-ups, managing referrals, and connecting individuals to resources
- Coach patients on remote patient monitoring adherence and address barriers to engagement
- Document clinical interactions accurately within the Babyscripts care management platform
Requirements
- Maintain a consistent schedule of approximately 24 hours per week with schedule flexibility
Qualifications
- Active and unrestricted United States RN license
- Multi-state compact nursing license
- Minimum of three years of clinical experience in OB/GYN, maternal health, remote patient monitoring, case management, or related fields
- Background in digital health, telehealth, or remote patient monitoring platforms
- Proficiency with digital health tools, electronic health records, and care management systems
- Bilingual fluency with a preference for Spanish
