Signify Health, operating as part of CVS Health, seeks a Part-time Nurse Practitioner to conduct In-Home Health Evaluations in Kirksville, Missouri. This remote position involves traveling locally and within licensed states to visit individuals in their private residences, providing personalized health assessments and bridging care gaps.
During approximately 45-minute appointments, the clinician engages one-on-one, reviews medical histories and medications, performs physical evaluations, and administers additional diagnostic tests like spirometry or diabetic eye exams if ordered by a health plan. Practitioners utilize an iPad and an intuitive workflow, and they are not responsible for prescribing medications, ordering lab tests, or altering current treatment regimens. The role requires conducting virtual visits as needed and navigating diverse home environments that may contain pets, secondhand smoke, or other allergens.
The position is anticipated to involve 29.5 hours per week. Qualified individuals must hold the necessary credentials and board certification to practice within the coverage area.
Responsibilities
- Perform in-home health evaluations and virtual visits for health plan members
- Review medical histories and current medications with patients
- Conduct physical evaluations and necessary diagnostic and preventive services such as spirometry and diabetic eye exams
- Provide health education and answer patient questions in a one-on-one setting
- Travel locally and within licensed states as required for visits
Requirements
