Job Description
Mable is seeking a Clinical Nurse for an initial six-month contract within its HomeMade team, which operates under the Attain Healthtech Group. HomeMade functions as an approved aged care provider focused on helping older adults maintain independence and self-manage service providers through technology and support teams. In this hybrid role, the Clinical Nurse utilizes telehealth to assist customers in staying safe at home.
This position involves triaging clinical concerns, conducting assessments, and providing expert guidance to support older individuals and their families. The temporary contract includes strong potential for extension or transition to permanent employment within the expanding team. Ideal candidates bring nursing expertise along with a warm and supportive approach to client care.
Responsibilities
- Monitor and triage the Clinical Inbox and wellbeing notes
- Complete clinical assessments and reviews including VATs and CNAs
- Support customers and families in making informed decisions
- Guide palliative care and advance care planning
- Respond to incidents and clinical escalations
- Contribute to continuous improvement and clinical quality initiatives
- Provide clinical input across the broader team and provider network
- Align practice with the Aged Care Act, Quality Standards, and the Clinical Governance Framework
Requirements
- Hold AHPRA Registration as a Registered Nurse
- Possess a First Aid certification
- Demonstrate strong clinical decision-making and case management skills
- Exhibit a warm, human approach and a passion for empowering others
- Display excellent communication and customer service skills
Qualifications
- Experience in Home and Community Care, ideally with aged care clients
- Familiarity with self-managed or consumer-directed care
